Nurse/Provider Sign-out

  1. Baby delivered early at 37 weeks and is now 2-days old.
  2. Mom on buprenorphine for 2 years and is doing well. No other medications/substances.
  3. Mom and baby will live with mom’s parents after discharge; father of baby is not involved.
  4. At times, baby feeds well, and other times, baby falls asleep soon after starting to feed. Baby has been sleeping and consoling well.
  5. Since the last NAS assessment 3-hours ago, baby was out of room for 30 minutes for newborn screening, and mom took a nap during this time. Mom is keeping the room calm and keeping the baby with her at all times, other than this one nap. Mom did not have any visitors present earlier, but her mother (baby’s grandmother) is here now. Grandmother just went to cafeteria to grab coffee.
  6. During the past day, vital signs are stable, and mom doing a combination of breast and and formula feeding. Baby breastfed 8 times and supplemented each feeding with 10mL formula. Baby had 2 voids, and 2 stools (green, slightly loose but not watery). Baby’s weight decreased 6% and 24-hour transcutaneous bilirubinometry (TcB) = 6.0. Baby jaundiced on chest. No tremors or high tone present at last assessment.
