Nurse/Provider Sign-out

  1. Jansi is a 2-day-old baby girl born at 41 weeks by vaginal delivery after a long induction. Mom is 27 years old; this is her first pregnancy. Mom on buprenorphine for 2 years and doing well. No other medications/substance exposures.
  2. During past day, baby breastfed 10 times, had 2 voids, and 4 stools. Weight decreased 7%, and today’s transcutaneous bilirubinometry (TcB) = 4.8.
  3. Lots of family visited during baby’s first day of life. Now, mom is alone. Baby’s dad is working long hours. Mom keeping TV on for company.
  4. Last assessment was 3-hours ago. Baby received all No for each ESC item and a 2 for Consoling Support Needed.
  5. After the assessment, baby was in the nursery for 1.5 hours, and a cuddler held the baby. Mom went for a walk while baby in the nursery. During this time, baby sucked a lot on hands/fingers, squirmed around, and was frantic by the time mom returned. Staff did not have any of mom’s milk to feed baby. Baby had 1 loose green stool, slightly watery, and no void.
